Such conditions include eyes with severe scarring\, corneal edema\, ocular trauma or a keratoprosthesis. \n“At my practice\, this represents a small percentage of patients\, between 5% and 10%\,” Chynn said. “And for general ophthalmologists\, it might be only 1% to 2% of patients.” \nOn the other hand\, many children cannot tolerate anyone touching their eyes\, so in the pediatric population\, perhaps 90% of patients would do better with the Diaton pen (BiCOM) than with Goldmann applanation tonometry\, Chynn said. \nNo contact with cornea\nAccording to Chynn\, the Diaton pen is a reputable tonometer that accurately measures IOP. Except for air-puff tonometry\, other devices that measure IOP\, including the gold-standard Goldmann applanation tonometry\, involve contact with the cornea. \nThere are inherent “dangers” in using a device that touches the cornea\, Chynn said\, citing corneal abrasion and infection. \n“The No. 1 reason non-ophthalmologists shy away from tonometry is that they are afraid of causing a corneal abrasion\,” he said. \nOn the other hand\, air-puff testing is not as accurate as Goldmann applanation tonometry. \n“I suspect that most ophthalmologists would agree that the air-puff is only good as a screening device\,” Chynn said. \nThe Diaton is useful as a screening device for non-ophthalmologists such as general practitioners\, internists or emergency department physicians because there is no contact with the cornea\, only the upper eyelid. “There is also no risk of infection or abrasion\,” Chynn said. \nUsing the device\nTo use the Diaton\, the patient either sits or lies on a chair\,  and then looks up at their extended thumb at an approximately 45° angle\, centering the eye. The clinician gently positions the tonometer at the edge of the eyelid\, so the pen is touching the edge of the limbus through the upper lid at the tarsus. The clinician then slightly lowers the pen\, with that releasing the light weight probe – takes 1 second. \nThe device is about the size of a highlighter and features a small digital readout. The test takes less than a minute for both eyes. \nDiaton Tonometer provides reliable measurements and makes it possible to diagnose glaucoma in its early stages\, allowing you to devise an appropriate care and treatment plan. Diaton is effective in obtaining IOP measurements for many challenging patient populations\, including: those with chronic conjunctivitis\, erosions\, edema and corneal dimness; people who have undergone corneal surgeries; and immobilized patients and children. Also\, patients do not need to remove their contact or scleral lenses. \nA busy general ophthalmologist who sees 50 patients a day could benefit from the Diaton\, especially those without a certified ophthalmic medical technician\, he said. \nFurthermore\, there are no consumables\, such as replacement tips or covers\, and no sterilization is required\, so the Diaton is less expensive\, Chynn said. SUMMARY:Unique Diaton Tonometer at Optometry's Meeting in Denver Colorado
DESCRIPTION:Innovative Diaton Scleral Tonometer at Optometry’s Meeting in Denver Colorado. \nBesides standard qualities of corneal tonometers\, Diaton is able to measure intraocular pressure (IOP) via sclera\, which us useful to be used on patients that are indicated to wear scleral lenses. Measuring IOP in patients with scleral lenses is becoming a standard of care. Diaton is unique and is only tonometry device on the market that is trans-scleral as well as trans-palpebral. \n \nA recent clinical study by Pacific University College of Optometry\, titled “Does IOP Increase During Scleral Lens Wear? ”  Concerns have been raised about potential unintended long term side effects of scleral lens wear. One of these side effects is a potential change in intraocular pressure (IOP). \nStudy Results: The Diaton measures IOP through the upper eyelid to the scleral surface. This allows clinicians to monitor IOP while scleral lenses are being worn. The correlation between IOP measurements taken with the Diaton and Goldmann were significant. 73.2% of observed Goldmann values fell within ±2 mmHg of the predicted Goldmann values predicted by our linear regression model. Immediately following lens application\, IOP increased (above baseline) in all subjects in both lens designs A and B. The increase in IOP was not significantly different between the two lens designs. The average increase in IOP for both lens designs was 5.5 mmHg. \nDiscussion: We observed that immediately following scleral lens application\, the IOP increased in all of the test subjects. The initial IOP increase averaged 5.5 mmHg. The pressure appears to remain elevated throughout the wearing time (average 7.0 mmHg increase from baseline prior to removal)\, then immediately returns to baseline following scleral lens removal. This suggests that the suction force beneath the lens is a primary contributor to the IOP changes. These suction forces may also be responsible for such phenomenon as conjunctival prolapse. However\, narrowing of the iridocorneal angle or compression of the trabecular meshwork and/or Schlemm’s canal due to the suction of the lens can not be ruled out as possible contributing factors to the IOP changes.
